Procedures for Applying for DASIL Membership

Initial membership dues payments must be made at the time of application. The dues amount for each level of membership is displayed below. Membership dues are based upon your national geographic location and professional status. Please select the one that reflects your current national residential status. If you are unsure whether your country is considered a developing country please check the Developing Countries list using this link.

As part of your application process, an online invoice will be sent which you can pay using one of the credit cards accepted by DASIL.
If you would prefer a blank application in PDF format, please email your request to Maureen Rosenthal.

Residents and Fellows-in-Training need to send, by separate email, a PDF file copy of a letter sent on institutional letterhead stationery signed by your program director indicating the hospital where training is taking place and when your training program will end. This material needs to be sent within 10 business days from the date your application is submitted. Send your director’s letter to Maureen Rosenthal.

In some instances, we may require that you send us a professional biographic profile. We need to receive your profile not more than 10 business days when you receive an email requesting this material.

In the event that payment in full and any required materials are not received by the deadline dates, your application will be removed and you will have to reapply for membership. Membership payments are not refundable unless your application is not approved.
